Master, Doctor Hoàng Khánh Toàn shared about the condition of sciatica in the program "Golden Guide for Health."
Sciatica is the largest nerve in the body, formed by nerve roots from the lumbar vertebrae L3, L4, L5, and S1. Sciatica is a common condition and is the most severe manifestation in the overall scenario of lower back pain. This symptom often appears suddenly and may resolve after a few weeks, but it can also persist for months or even years, significantly impacting the ability to work and the quality of the spine.
The most common cause is a herniated disc due to heavy labor and poor posture. Other causes, such as spinal degeneration, infections, or tumors pressing on the nerves, also lead to sciatica symptoms.
Typical symptoms of sciatica include:
Lower back pain that radiates down the buttocks, thighs, calves, and feet along the path of the sciatic nerve.
Mechanical pain that worsens when coughing, sneezing, bending, or standing or sitting for long periods, and improves with rest.
Limited movement of the spine, making daily activities difficult for the patient.
May be accompanied by sensations of crawling, numbness, or a sharp prickling feeling on the outer edge of the foot, extending to the big toe (lumbar root 5), or the heel or little toe (sacral root 1).
Can also be associated with symptoms in the lower abdominal region, affecting urination and bowel movements due to widespread nerve root damage in the cauda equina.
Chronic cases may lead to muscle atrophy in the affected nerve areas, spinal deformities, and abnormal posture or gait.
TREATMENT METHOD FOR SCIATICA PAIN
Approach to Treatment in Western Medicine
Sciatica pain is typically treated with the following types of medications:
When conservative treatment (usually after 3-6 months of proper medical treatment) does not result in improvement (e.g., no pain relief or muscle atrophy), and if the patient shows signs of nerve root compression such as dysfunction of the anal sphincter, paralysis of the legs, or rapid muscle atrophy, then surgical intervention is considered to relieve nerve compression in the spinal region.
Advantages of Western Medicine:
Provides rapid pain relief, offering immediate comfort for the patient
Convenient and easy to obtain
Disadvantages:
Can cause stomach pain
Long-term use of painkillers can lead to liver and kidney damage
Tolerance to the drug may develop, requiring increased dosages over time
Long-term use of neuropathic pain medications can affect the nervous system and mental health
Paralysis and reduced mobility can occur after surgery
When treated with pain relievers that temporarily eliminate pain, the patient may be misled into resuming physical activities, worsening the condition. Due to these undesirable side effects, Traditional Chinese Medicine (TCM) is gradually becoming a long-term treatment option for patients with sciatica pain.
Sciatica pain is described in the context of TCM as part of "Wind-Damp" syndrome, known as "Yao Guo Tong" or "Zuo Gu Feng."
Support for Treating Sciatica Pain with TCM Methods
In TCM, sciatica pain is believed to be caused by external pathogens (wind, cold, damp, heat) invading the body, disrupting the flow of vital energy (Qi) and blocking the meridians (especially the gallbladder and bladder meridians). It can also result from a weakened immune system, dysfunction of the internal organs, particularly the liver and kidneys, or from blood stagnation and Qi stagnation blocking the meridians, causing pain.
TCM theory holds that: The kidneys store essence, which governs bones and marrow; the liver stores blood, which nourishes tendons; the smooth circulation of Qi and blood throughout the body nourishes internal organs and protects against external pathogens. TCM treatment for sciatica focuses on diagnosis and treatment based on the principle: "Treat the root cause of the disease; for acute diseases, treat the symptoms first; for chronic conditions, treat the root cause."
"Hoạt Lạc Chỉ Thống" (Active and Pain Relieving Decoction) is a classical TCM formula often used for treating sciatica. The medicinal ingredients are combined according to the principles of monarch, minister, assistant, and envoy.
Specific ingredients:
✔ Du Huo (Monarch): Bitter and slightly warm, entering the kidney, liver, and bladder meridians. It helps expel wind, eliminate dampness, relieve pain, and treat lumbar and knee pain caused by wind-damp conditions.
✔ Ngưu Tất (Minister): Bitter and sour, neutral, enters the kidney and liver meridians. It promotes blood circulation, relieves pain, strengthens tendons and bones, and is useful for back and knee pain and muscle fatigue.
✔ Tang Ký Sinh (Minister): Bitter, neutral, enters the kidney and liver meridians. It strengthens the kidney and liver, nourishes tendons and bones, and helps relieve sciatica and peripheral nerve pain.
✔ Uy Linh Tiên (Assistant): Bitter, warm, enters the bladder meridian. It helps expel wind and dampness, relieve pain, and treat joint and tendon stiffness.
✔ Đan Sâm (Assistant): Bitter, slightly cool, enters the heart and liver meridians. It invigorates blood circulation, clears heat, and relieves pain. It is used for blood stasis, chest pain, insomnia, and irritability.
✔ Chỉ Xác (Envoy): Bitter, spicy, slightly cool, enters the spleen and stomach meridians. It helps transform phlegm and relieve digestive issues.
✔ Phòng Phong (Envoy): Spicy, sweet, warm, enters the lung, spleen, and bladder meridians. It helps expel wind, relieve cold, and reduce pain from wind-damp conditions.
✔ Trần Bì (Envoy): Bitter, spicy, warm, enters the lung and spleen meridians. It helps regulate Qi, improve digestion, and clear dampness.
✔ Xuyên Khung (Envoy): Spicy, warm, enters the liver, gallbladder, and heart meridians. It moves Qi, invigorates blood, and relieves pain. It is effective for headaches and joint pain.
✔ Tế Tân (Envoy): Spicy, warm, enters the heart, lung, liver, and kidney meridians. It expels wind, dispels cold, promotes Qi circulation, and alleviates blood stagnation.
This formula is designed to expel wind, warm and eliminate cold-dampness, nourish the liver and kidney, and relieve pain by promoting the smooth flow of Qi and blood. It treats both the external causes of the disease and nourishes the internal organs, helping relieve sciatica pain and promoting stronger bones and muscles, ultimately improving overall health.
