Kidney Stones
Kidney stones are solid stones formed from mineral crystals dissolved in urine, which settle in the renal pelvis of the patient. Kidney stones usually originate in the kidneys themselves. However, they can develop anywhere along the urinary tract of the patient and can even move back up to the kidneys (though this is rare). The parts affected may include:
Kidneys
Ureters
Bladder
Urethra
Sỏi thận là những sỏi rắn được hình thành từ các tinh thể khoáng chất hoà tan trong nước tiểu, lắng đọng tại đài bể thận của bệnh nhân
Kidney Stones
When kidney stones move or become obstructed, they can cause significant pain and discomfort for the patient, and they can also lead to serious and dangerous complications. The causes of kidney stones vary depending on the type of stone.
Causes of Kidney Stones
Not all kidney stones are made up of the same type of crystals. The different types of kidney stones include:
Các loại sỏi thận
Calcium Stones
Calcium stones are the most common type. They are typically made of calcium oxalate (although they can also include calcium phosphate or maleate). A diet that limits foods high in oxalate can reduce the risk of developing calcium oxalate stones. Foods rich in oxalate include:
French fries
Peanuts
Chocolate
Beets
Spinach
However, although calcium is the main component of the stone, getting enough calcium in the patient’s diet may actually help prevent stone formation.
Uric Acid Stones
Uric acid stones are more common in men than in women. They can form in the kidneys of individuals with metabolic disorders like hyperuricemia (gout) or those undergoing chemotherapy.
These stones form when urine becomes too acidic. A diet rich in purine can increase the acidity in urine. Purine is a colorless substance found in animal proteins such as fish, shellfish, and red meat.
Struvite Stones
Struvite stones are most commonly found in women who have recurrent urinary tract infections (UTIs) that are not properly treated. These stones can be large and cause urinary tract obstruction. They are the result of kidney infections. Treating the underlying infection can prevent the development of struvite stones.
Cystine Stones
Cystine stones are very rare. They occur in both men and women with a genetic disorder called cystinuria. In this condition, cystine, a naturally occurring acid in the body, is abnormally excreted from the kidneys into the urine.
Symptoms of Kidney Stones
Kidney stones that move and cause obstruction are known to trigger intense renal colic pain. Symptoms of kidney stones may not occur until the stone starts moving down the ureter. This severe pain is called renal colic. The patient may experience pain in one side of the back or abdomen that radiates to the back, although it is rare to have pain symmetrically on both sides.
Sỏi thận di chuyển và gây tắc nghẽn được biết là nguyên nhân gây ra những cơn đau quặn thận dữ dội
In men, pain originating from the kidneys can radiate to the groin area. Renal colic comes and goes but can be intense. Those experiencing renal colic often exhibit restlessness due to the severe pain, which occurs in episodes.
Other symptoms of kidney stones may include:
Blood in the urine (urine that is red, pink, or brown)
Vomiting
Nausea
Urine that changes color or has a foul odor
Chills
Fever
Frequent urge to urinate (frequent urination)
Passing only small amounts of urine
In the case of a small kidney stone, the patient may not experience any pain or symptoms as the stone passes through the urinary tract.
Complications of Kidney Stones
Not all stones remain in the kidneys. Sometimes, they travel from the kidneys down to the ureters. The ureters are very small and delicate, and the stones may be too large to pass straight down the ureter to the bladder.
Stones moving down the ureters can cause spasms and irritation or blockages in the ureters. This can result in blood in the urine.
Máu trong nước tiểu
Sometimes, stones can block the flow of urine. This is called urinary tract obstruction. Blockages in the urinary tract can lead to kidney infections and kidney damage due to hydronephrosis, affecting both the renal tubules and glomeruli.
Risk Factors for Kidney Stones
The greatest risk factor for kidney stones is not drinking enough water, which causes the body to produce and excrete less than 1 liter of urine per day. This is why kidney stones are common in premature infants with kidney issues. However, kidney stones are also very common in people aged 20 to 50.
Other factors that can increase the risk of developing kidney stones include: In the United States, white people are more likely to develop kidney stones than black people.
Gender also plays a significant role. According to the National Institute of Diabetes and Digestive and Kidney Diseases (NIDDK), more men develop kidney stones than women.
A history of kidney stones can increase the risk of recurrent stones. A family history of kidney stones also increases the risk.
Other risk factors include:
Dehydration
Obesity
A diet high in protein, salt, or glucose
Hyperparathyroidism
Stomach surgery
Inflammatory bowel disease, which increases calcium absorption
Medications such as the diuretic triamterene, anticonvulsants, and calcium-based antacids
Treatment Options for Kidney Stones
Treatment is tailored according to the type of stone. Urine may be tested, and stones may be collected for evaluation.

Drinking six to eight cups of water per day increases urine flow. Those who are dehydrated or experiencing severe nausea and vomiting may require intravenous fluids.
Other treatment options include:
Medications
Pain relief may require the use of anesthetics. The presence of infection requires treatment with antibiotics. Other medications include:
Allopurinol (Zyloprim) for uric acid stones
Thiazide diuretics to prevent calcium stone formation
Sodium bicarbonate or sodium citrate to make urine less acidic
Phosphate solutions to prevent calcium stone formation
Ibuprofen (Advil) for pain relief
Acetaminophen (Tylenol) for pain relief
Naproxen sodium (Aleve) for pain relief
Traditional Medicine for Kidney Stones Treatment
According to Traditional Chinese Medicine (TCM), diseases related to the urinary tract, such as urinary tract infections, urinary stones, and lactosuria, all share common symptoms of "Lâm." Kidney disease is referred to as "Thạch Lâm", which means stones in the kidney. It has main symptoms like frequent urination, difficulty urinating, painful urination, and lower abdominal pain. Kidney stones belong to the "Lâm" category in TCM, referring to stones formed in various parts of the body.
Based on clinical symptoms, traditional medicine categorizes "Lâm" into five types: Heat Lâm, Stone Lâm, Blood Lâm, Gao Lâm, and Lao Lâm. Stone Lâm is understood as stones in the kidney, formed due to the accumulation of heat and dampness in the lower body that leads to the precipitation of urinary substances, causing the disease. Symptoms of Stone Lâm include lower abdominal pain, cramping pain in one side of the lower back, pain radiating to the lower abdomen and genital area, and painful and difficult urination. Patients may also experience sluggish urination, sometimes with cloudy or blood-tinged urine, or with the passage of small stones.
To treat Stone Lâm, it is necessary to address the root cause, which is heat and dampness in the lower body. It is important to prevent heat from invading the body, which leads to urinary tract infections, heat accumulation in the bladder, and the depletion of body fluids. Over time, this can lead to the formation of stones. Therefore, TCM emphasizes treating the root cause, tonifying the kidneys, nourishing the blood, expelling pathogenic factors, and improving the function of the internal organs to enhance the patient's overall health.
The prescription Lục vị địa hoàng hoàn combined with Kim tiền thảo is used to tonify the kidneys, clear heat, promote urination, and dissolve stones. Specifically:
Kim Tiền Thảo: Clears heat, dispels dampness, promotes urination, and expels stones. It is used to treat urinary tract stones and gallstones.
Thục địa: Nourishes the yin and nourishes the blood.
Sơn thù: Tonifies the liver and kidneys.
Phục linh: Promotes urination, eliminates dampness, strengthens the spleen, and calms the mind.
Sơn dược: Nourishes the stomach and generates fluids.
Mẫu đơn: Clears heat, cools the blood, activates blood circulation, and removes stasis.
The company MTA Pharmaceutical Joint Stock Company has applied the essence of this formula using modern technology that meets GMP standards to produce MTA Trạch Tả, which helps clear heat, promote urination, and reduce the risk of kidney stone formation, as well as stones in the ureters and bladder. It is very effective for those experiencing frequent urination and painful urination due to kidney stones, urinary stones, or bladder stones.

Prevention of Kidney Stones
Drinking an adequate amount of water is a key preventive measure. Doctors recommend drinking enough water to produce at least 2.0 liters of urine each day. Increasing the amount of urine excreted by the patient helps prevent the formation of crystals in the kidneys.
Uống nhiều nước một cách thích hợp là một biện pháp phòng ngừa chính
Patients can replace beer, lemon soda, and fruit juices with water to help increase fluid intake. If the stones are related to low citrate levels, citrus fruit juices, which are rich in citrate, can help prevent the formation of stones.
Eating foods rich in oxalates in moderation and reducing the intake of salt and animal protein can also lower the risk of kidney stones.
Doctors may prescribe medications to help prevent the formation of calcium and uric acid stones. If the patient has had kidney stones or is at risk of developing them, they should consult with their doctor to discuss the best preventive measures.
