BENIGN PROSTATIC HYPERPLASIA (BPH) IN TRADITIONAL MEDICINE
What is Benign Prostatic Hyperplasia? Benign prostatic hyperplasia (BPH) is a common condition in men, especially those in middle age and older. It is a non-cancerous enlargement of the prostate gland.
According to Traditional Oriental Medicine, BPH is classified into three main syndromes:
Bladder Heat Accumulation (Bàng quang tích nhiệt)
Bladder Qi Stagnation (Bàng quang ứ trở)
Kidney Yang Deficiency (Thận dương hư suy)
Let’s explore BPH through the lens of Traditional Oriental Medicine with MTA.
What is Benign Prostatic Hyperplasia (BPH)?
The prostate is a male reproductive organ located just below the bladder and surrounding the urethra, the tube through which urine flows. According to Traditional Chinese Medicine (TCM), the prostate is responsible for producing the fluid in semen, a milky fluid that nourishes and transports sperm out of the penis during ejaculation (orgasm). Typically, the prostate weighs 15-20g and develops from puberty until the age of 25. After age 40, the prostate may enlarge, causing urinary tract issues, and this condition is considered benign prostatic hyperplasia (BPH). Men over 50 are often affected by this condition. Statistics show that over 90% of men in their 80s suffer from BPH.
In Traditional Chinese Medicine (TCM), BPH is categorized under various syndromes such as ‘Lâm Chứng,’ ‘Bạch Trọc,’ ‘Huyết Lâm,’ ‘Bạch Dâm,’ ‘Niệu Tinh,’ ‘Tinh Trọc,’ ‘Lâm Trọc,’ ‘Lao Lâm,’ and ‘Khí Lâm’.
Causes of Benign Prostatic Hyperplasia
According to TCM, the causes of BPH include:
Damp-Heat Accumulation in the Lower Body: This condition can result from external damp-heat toxins, or the transformation of cold pathogens into heat. The heat and dampness compete, accumulating in the lower body and stagnating in the prostate, which causes Qi stagnation, blood congestion, and obstruction of the meridians. The accumulation of damp-heat in the bladder can prevent Qi transformation, leading to prostate enlargement. Additionally, improper diet and habits like eating spicy, greasy, and sweet foods can affect the Spleen and Stomach, causing irregular digestion, internal damp-heat accumulation, and eventually impacting the prostate.
Yin Deficiency and Excessive Heat: Excessive sexual activity depletes essence, weakens kidney essence, and results in Yin deficiency, leading to excessive fire. This fire then damages the essence and obstructs the prostate, causing stagnation and the generation of heat.
Spleen Qi Deficiency: Overexertion, prolonged sitting, excessive thinking, or irregular eating habits can weaken the Spleen, which fails to transform dampness. This causes stagnation in the prostate, leading to the formation of damp-heat that cannot be cleared.
Kidney Essence Deficiency: Older age, physical weakness, or chronic illness can deplete kidney essence, weaken the Kidney Qi, and result in a decline in Yang, which affects the ability to retain essence, causing leakage.
Stagnation in the Prostate: Long-term damp-heat retention and excessive fire accumulation result in stagnation of semen and Qi, which obstructs the flow in the prostate, leading to the condition.
Lung Heat and Qi Stagnation: Heat from the lungs can accumulate and obstruct the water pathways, further causing the heat to descend to the bladder.

Benign Prostatic Hyperplasia Symptoms
Benign prostatic hyperplasia (BPH) can cause many uncomfortable symptoms. Here are some typical symptoms of this condition:
– Difficulty urinating (urinary retention): The patient feels the urge to urinate but finds it very difficult. It may take a while to start urinating, and there is often a feeling of weak urine flow, sometimes accompanied by frequent dribbling.
– Urinary incontinence: This occurs when urine is released involuntarily, especially during sleep. Sometimes, this may even happen during the day.
– Intermittent urination: This condition is often associated with the formation of bladder stones, which cause sudden interruptions in the flow of urine. Symptoms may worsen when exposed to cold or after consuming alcohol.
– Frequent urination: The frequency of urination increases by 3 to 4 times a day compared to the average. The intervals between urination become shorter, and there is a constant feeling of urgency to urinate.
