Chronic Colitis
What is Chronic Colitis?
Chronic colitis is the result of recurrent acute inflammation episodes. The disease may be diffuse or localized in the mucous membrane of the colon, causing ulcers in this area.
Once colitis progresses to the chronic stage, it becomes very difficult to treat. At mild stages, individuals may experience bleeding in the colon. However, as the disease worsens, more serious issues such as ulcers, bleeding, or abdominal abscesses may arise.
The Incidence of Colitis in Our Country
The incidence of colitis in our country has reached 20% of the population, according to statistics from the Ministry of Health, and the rate is increasing rapidly. Therefore, the treatment of chronic colitis is becoming increasingly important.
What Causes Chronic Colitis?
Chronic colitis can be caused by various factors. However, in most cases, it is the result of experiencing other types of colitis for a prolonged period.
The reasons we may develop chronic colitis include:
Inadequate Treatment of Acute Colitis: Acute colitis occurs due to infections by parasites, bacteria, fungi, or poisoning. If the acute condition lasts for 3 months or longer, it may develop into chronic colitis.
Underlying Conditions: If you have other colon diseases, such as colon tuberculosis or Crohn’s disease, and do not treat them promptly, you are at high risk of developing chronic colitis as the colon mucosa becomes damaged over time.
Poor Diet: Eating unhygienically can lead to infections from harmful bacteria in the intestines. Eating at irregular hours or in an unscientific manner disrupts the colon’s function and, over time, may lead to chronic colitis.
Excessive Use of Antibiotics: Overuse of Western medicine, particularly antibiotics, can erode the colon mucosa, making it more vulnerable to inflammation and ulcers.
Symptoms of Chronic Colitis
Depending on the underlying cause of the disease and its severity, you may experience different symptoms. However, compared to acute colitis, the symptoms of chronic colitis are usually more intense and persistent.
Here are some common symptoms of chronic colitis:
Abdominal Pain: Pain is usually felt in the left or right lower abdomen. It may vary from dull to intense pain.
Frequent Urge to Defecate: There is often a sensation of urgency when going to the bathroom.
Disrupted Bowel Movements: People with chronic colitis may experience frequent urges to defecate, diarrhea, or constipation, and sometimes both.
Abnormal Stool: The stool may be sticky, and occasionally there may be traces of blood.
Additionally, systemic symptoms may accompany the condition, such as:
Fatigue
Loss of appetite
Unexplained weight loss
Joint pain
Insomnia
Anemia
Pale skin
Dizziness
In severe cases, chronic colitis may also lead to memory problems.
Diagnosis of Chronic Colitis
Like typical colitis, the diagnosis of chronic colitis relies on clinical symptoms. The doctor will also review your medical history and lifestyle, including diet and habits.
Once an initial diagnosis is made, the doctor will likely perform a colonoscopy to monitor the inflammation and the extent of the damage. They may also take tissue samples for biopsy, which helps identify the underlying cause of the disease and screen for potential cancer.
Other tests that may be ordered include:
Blood tests
Stool tests
Urine analysis
Abdominal X-ray
CT scan
These tests will help the doctor confirm the diagnosis and develop an appropriate treatment plan.
Treatment of Chronic Colitis
Western Medicine Treatment
The main approach in Western medicine is to use medications to reduce and manage symptoms:
Antibiotics: Used for bacterial infections, to control bacterial spread, and manage digestive and bowel irregularities.
Corticosteroid Anti-inflammatory Drugs: These are used to reduce inflammation, especially in the early stages of treatment.
Antiparasitic and Antifungal Medications: If the condition is caused by parasites or fungi.
Pain Relievers and Antispasmodics: Used to alleviate pain and reduce spasms in the intestines.
Surgical Treatment for Chronic Colitis
If a tumor or cancerous growth is detected, or if the disease progresses to a stage where it cannot be controlled with medication, the patient will be advised to undergo surgery.
Traditional Chinese Medicine (TCM) Approach
In Traditional Chinese Medicine (TCM), chronic colitis is considered to be linked to various conditions such as dysentery and abdominal pain. The underlying causes of the disease are believed to be due to four main factors:
External Pathogenic Factors (Ngoại tà lục lâm): Infection from external sources, including bacteria, viruses, or other harmful agents.
Improper Diet (Ẩm thực bất điều): Unbalanced or irregular eating habits.
Emotional Stress (Thất tình nội thương): Prolonged stress, anxiety, or emotional strain can contribute to the development of digestive disorders.
Spleen and Stomach Deficiency (Tỳ vị tố hư): Weakness in the spleen and stomach function, which leads to poor digestion and prolonged illness.
To treat chronic colitis effectively in TCM, it is important to address the root cause of the disease, while also nourishing the spleen and stomach, harmonizing the liver and stomach, and ultimately improving digestive health.
Herbal Remedy: Cát căn Hoàng cầm Hoàng liên thang
This herbal formula is used to treat chronic colitis by clearing heat, resolving dampness, and soothing the intestines. The main ingredients include:
Cát căn: Used as the main ingredient to expel pathogens and relieve the body.
Hoàng cầm and Hoàng liên: These herbs act as secondary ingredients, clearing heat and drying dampness to reduce inflammation and pain.
Cam thảo: Used to harmonize the formula and relieve digestive discomfort.
This formula is effective for treating both acute and chronic colitis, as well as digestive disorders like bloating, irregular bowel movements, and chronic diarrhea or constipation.
