Chronic colitis is the result of repeated acute infections that cause inflammation over time. The condition may be diffuse or localized on the mucosal lining of the large intestine, leading to ulcers in the area.
Once colitis progresses to the chronic stage, treatment becomes significantly more challenging. In mild cases, you may experience rectal bleeding. However, as the disease worsens, more serious issues such as ulcers, bleeding, or abdominal abscesses may arise.
Prevalence of Chronic Colitis in Vietnam
According to statistics from the Ministry of Health, the rate of chronic colitis in Vietnam is up to 20% of the population, with a rapid increase. Therefore, the treatment of chronic colitis is becoming increasingly important.
What Causes Chronic Colitis?
Chronic colitis can be caused by many factors, but it is most commonly the result of prolonged inflammation from other forms of colitis.
Here are some reasons why chronic colitis may develop:
Inadequate Treatment of Acute Colitis: Acute colitis is caused by infections from parasites, bacteria, fungi, or toxins. If acute colitis lasts for more than 3 months, it may develop into chronic colitis.
Other Underlying Conditions: If you suffer from other conditions like tuberculosis of the colon or Crohn's disease, and do not treat them promptly, you are at a high risk of developing chronic colitis. This is because the mucosal lining of the colon may have been damaged over a long period.
Poor Diet: Eating unhygienic food can lead to harmful bacteria infecting the intestines, and irregular eating habits or unhealthy diets can disrupt the functioning of the colon, which may eventually lead to chronic colitis.
Excessive Use of Antibiotics: The overuse of Western medicine, particularly antibiotics, can erode the mucosal lining of the colon, leading to inflammation and ulcers.
Symptoms of Chronic Colitis
The symptoms of chronic colitis can vary depending on the underlying cause and the severity of the condition. However, compared to acute colitis, the symptoms of chronic colitis are usually more persistent and severe.
Here are some common symptoms of chronic colitis:
Abdominal Pain: Pain is often felt in the left or right lower abdomen. The pain may be dull or sharp.
Frequent Urge to Defecate: Often feeling the urge to pass stools, accompanied by difficulty.
Bowel Movement Disturbance: Frequent urges to defecate, diarrhea, or constipation, and sometimes both.
Abnormal Stool: Stools may be sticky, sometimes with traces of blood.
In addition to these local symptoms, individuals may experience general symptoms such as:
Fatigue
Loss of appetite
Unexplained weight loss
Joint pain
Insomnia
Anemia
Pale skin
Dizziness
In more severe cases, chronic colitis can also contribute to memory impairment.
Diagnosis of Chronic Colitis
As with typical colitis, doctors diagnose chronic colitis based on the symptoms presented. They will also review the patient’s medical history and dietary and lifestyle habits.
After the initial diagnosis, doctors usually perform a colonoscopy to observe the inflammation and the degree of damage to the colon. A biopsy may also be taken to help determine the cause of the disease and screen for cancer.
Additional tests that may be conducted include:
Blood Tests
Stool Tests
Urinalysis
Abdominal X-ray
CT Scan
These tests help the doctor confirm the diagnosis and design an appropriate treatment plan.
Treatment of Chronic Colitis
Western Medicine Treatment
The main treatment for chronic colitis in Western medicine is medication aimed at reducing and controlling symptoms.
Antibiotics: Used in cases of bacterial infection to reduce bacterial spread and control digestive disturbances.
Corticosteroids: Anti-inflammatory medications commonly used during the initial stages of treatment to reduce inflammation.
Antiparasitic or Antifungal Medications: Used to treat parasitic or fungal infections.
Pain Relief and Antispasmodic Medications: To alleviate pain and muscle spasms in the intestines.
Treatment of Chronic Colitis with Surgery
If a tumor is detected or the disease progresses to a stage that cannot be managed with medication, surgery may be recommended for the patient.
Traditional Medicine Treatment for Chronic Colitis
In Traditional Chinese Medicine (TCM), chronic colitis is categorized under disorders like dysentery and abdominal pain. The causes of chronic colitis are considered to be influenced by four main factors:
External Pathogens (Ngoại tà lục lâm): Infection or external pathogens affecting the body.
Irregular Eating Habits (Ẩm thực bất điều): Unhealthy or irregular eating habits that disrupt digestion.
Spleen and Stomach Deficiency (Tỳ vị tố hư): A weakened spleen and stomach function, which leads to prolonged illness.
To effectively treat chronic colitis, it is essential to address the root causes of the disease and simultaneously strengthen and harmonize the spleen and stomach functions to boost digestive health.
Traditional Medicine Formula
A traditional formula that includes Thảo đậu khấu (an herb with a pungent taste, warm nature, and a connection to the Spleen and Stomach) is used to treat abdominal pain, indigestion, and vomiting. This herb is combined with other valuable herbs such as:
Mộc hương (A medicinal herb that promotes the movement of Qi and relieves pain, strengthens the spleen, and aids digestion)
Hậu phác (A medicinal herb used for its digestive and detoxifying effects)
Ích trí nhân (Herb used to enhance digestion)
Thanh bì (Helps regulate the digestive system)
Mạch nha (Used to treat digestive problems)
Bạch truật (An herb known for its ability to strengthen the spleen and improve digestion)
Sa nhân (Helps with digestive issues)
Together, these herbs work to support the spleen and stomach, regulate the liver and stomach, and treat chronic colitis.

Energy: 30 – 35 Kcal/kg/day depending on the patient.
For fishy foods like shrimp, crab, fish, eggs, eat in moderation and consume them immediately after preparation.
Ripe fruits: Sapodilla, banana, sweet mango.
When constipated: Reduce fats, increase fiber (especially soluble fiber like pectin, inulin).
When having diarrhea: Avoid insoluble fiber such as cellulose to prevent friction in the intestines.
Foods and Drinks to Avoid:
Eggs, milk with lactose, fried fermented sausages, fatty meats, black beans, raw onions, pickled vegetables, alcohol, coffee, carbonated drinks, as these can cause bloating and discomfort.
Avoid foods high in lactose, such as cow's milk, honey, and sorbitol (found in some candies) to prevent diarrhea, as patients may have poor absorption of these sugars, leading to bloating, flatulence, and diarrhea.
Avoid hard foods such as raw vegetables, corn kernels, bamboo shoots, etc., as they can negatively affect ulcers. Food should be steamed or boiled, with minimal frying or saucing.
Avoid processed foods.
Do not drink alcohol, coffee, or smoke.
