Urinary Retention (Bí Tiểu)
Urination is the body's way of excreting waste. When a person experiences urinary retention, it can be very uncomfortable and can significantly affect daily life, work, and even threaten one's life.
What is Urinary Retention?
Normally, the human body can urinate voluntarily, with a harmonious contraction of the bladder and relaxation of the bladder neck. Urinary retention occurs when the bladder fills with urine but the person cannot urinate, or can urinate but the bladder does not empty completely, causing frequent feelings of needing to urinate.
Urinary retention mainly affects adults, both men and women, with men being 10 times more likely to experience this condition than women, especially men between the ages of 40 to 80.
Urinary retention is categorized into two types:
Chronic Urinary Retention: This occurs over a long period. The person can urinate but the bladder does not empty completely. Chronic urinary retention often does not show clear symptoms in the early stages, so many patients may not notice the problem until it becomes more severe. Chronic urinary retention can lead to serious complications.
Acute Urinary Retention: This happens suddenly. The person feels the urge to urinate but cannot. Acute urinary retention causes abdominal discomfort and pain. If the person does not relieve the urine, it can affect their life, and they should seek immediate medical attention.
Causes of Urinary Retention
Typically, when the bladder contains around 250–350 ml of urine, it stimulates the feeling of needing to urinate. The amount that triggers this urge can vary from person to person. In individuals with urinary retention, the urine is already at a certain level in the bladder but cannot be excreted.
There are many causes of urinary retention. If the bladder wall cannot contract sufficiently, it may be due to a loss of communication with the nervous system that controls urination. Causes of this loss of communication include: spinal cord injury, pelvic fractures, bladder diseases (e.g., cystitis, bladder stones, bladder diverticula, bladder sclerosis, sclerosis of the bladder neck, etc.).
In some cases, bladder stones can move to the bladder neck and block the passage, obstructing the flow of urine. Chronic urethritis leading to fibrosis, narrowing of the urethra due to infection, bacteria, or urethral sclerosis can also cause urinary retention.
In men, urinary retention can also be caused by prostate diseases that press on the bladder neck, such as prostatitis or prostate enlargement. In women, urinary retention can be caused by pelvic diseases that compress the bladder, such as ovarian cysts or fibroids.
Some cases of temporary urinary retention may be due to psychological factors, such as sitting for too long or being in a crowded space.
Symptoms of Urinary Retention
Chronic urinary retention may not show specific symptoms in the early stages. It is usually only noticed when the condition worsens.
Common symptoms of urinary retention include:
Pain and pressure in the lower abdomen, bladder area, and pubic region.
Persistent discomfort and restlessness.
Frequent urge to urinate but inability to do so.
Is Urinary Retention Dangerous?
The symptoms of urinary retention can cause the patient to be restless for hours, even daily, leading to poor mental health, insomnia, fatigue, and stress.
If left untreated or if urinary retention recurs frequently, leading to urine retention, it can cause bladder infections. Infected urine that travels upward can lead to kidney infections and kidney failure. Therefore, patients need to seek medical care and get treatment as soon as possible.
Treatment for Urinary Retention
Patients with urinary retention need to have their bladder emptied to release the waste from the body, relieving discomfort and pressure. Afterward, the patient should undergo a thorough examination and necessary tests to diagnose the cause and determine appropriate treatment. During this process, medical instruments must be sterilized to avoid urinary tract infections.
It is essential to treat the underlying causes of urinary retention. Many causes require long-term treatment, so patients need to be patient and follow the doctor's instructions to achieve effective results.
Traditional Medicine Approach
In Traditional Chinese Medicine (TCM), urinary retention is categorized as "Long Bi" or "Lung Bi." "Lung" refers to infrequent urination or difficulty urinating. "Bi" refers to the inability to urinate, even a drop.
