PHONG PHONG – JOINT RELAXATION – SCIATIC PAIN
The sciatic nerve is the largest nerve in the body, formed by nerve roots from the lumbar vertebrae L3, L4, L5, and S1. Sciatica is a common condition and represents the most severe manifestation in the general context of lumbar spine pain. This condition often appears suddenly, may resolve after a few weeks, but can also last for months or even years, significantly impacting labor capacity and spinal quality of life.
The most common cause is a herniated disc due to heavy labor and poor posture. Other causes, such as spinal degeneration, infections, or compressive tumors, can also cause sciatica symptoms.
Typical Symptoms of Joint Pain – Sciatica:
Lower back pain radiating to the buttocks, thighs, and feet along the path of the sciatic nerve.
Mechanical pain, which worsens with coughing, sneezing, bending, or prolonged sitting or standing, and improves with rest.
Restricted movement in the spinal region, making daily activities difficult for the patient.
Possible sensations of tingling, numbness, or needle-like pain on the outer edge of the foot, crossing over the top of the foot to the big toe (lumbar root 5), or at the heel or little toe (sacral root 1).
May be accompanied by pain in the lower abdomen, affecting urination or bowel movements due to extensive damage to the nerve roots of the cauda equina.
Long-term conditions may lead to muscle atrophy in the affected nerve region, spinal deformities, and asymmetric posture, causing abnormal gait.
METHODS OF TREATING SCIATIC NERVE PAIN
Western Medicine Treatment Approach
The treatment for sciatic nerve pain generally involves the use of the following medications:
When medical treatment does not yield results (usually after 3-6 months of appropriate medical treatment with no pain relief and/or muscle atrophy), patients showing signs of nerve root compression, such as dysfunction of the sphincter, paralysis of the legs, or rapid muscle atrophy, will be considered for surgical intervention to release the compression in the spinal region.
Advantages of Western Medicine:
Quick pain relief, providing immediate comfort to patients.
Convenient and easy to purchase.
Disadvantages:
Stomach pain
Liver and kidney damage due to prolonged use of painkillers
Drug tolerance, requiring dose increases with long-term use
Potential impact on nerves and mental health when neuropathic pain relievers are overused
Paralysis or reduced movement may occur after surgery.
When pain relief medications temporarily relieve the pain, they may trick the patient into increasing movement, which can worsen the condition. Due to these unwanted side effects, Traditional Chinese Medicine (TCM) is gradually becoming a preferred long-term choice for patients with sciatica.
Sciatica is described within the scope of the TCM condition known as "Tý" and is given specific names such as Yêu Cước Thống and Tọa Cốt Phong.
Supporting Treatment for Sciatic Pain Using Traditional Chinese Medicine (TCM)
According to TCM, sciatica is caused by external pathogens (wind, cold, dampness, heat) that exploit weaknesses in the body’s defensive system, allowing them to invade and block the meridians (primarily the gallbladder and bladder meridians). It can also be caused by a weakness in the body's vital energy (Qi), dysfunction of the organs, especially the liver and kidneys, or blood stasis and Qi stagnation, which block the meridian flow and cause pain.
The TCM theory asserts that: The kidneys store essence, which governs the bones and marrow; the liver stores blood, which governs the tendons; Qi and blood circulate throughout the body, nourishing the organs and protecting the body from external pathogens. TCM treats sciatica by focusing on diagnosis and treatment principles such as: “Treat the root of the disease for chronic conditions, treat the symptoms for acute conditions, and treat the root when conditions have been present for a long time.”
Hoạt Lạc Chỉ Thống Thang is a classic herbal formula recorded in ancient medical texts, commonly used for sciatica. It consists of herbs that are harmoniously combined according to the principles of Jun – Chen – Zuo – Shi (Emperor, Minister, Assistant, and Envoy).
Specific Herbs:
Độc hoạt (Jun): Pungent, bitter, slightly warm, and acts on the kidney, liver, and bladder meridians. It helps dispel wind, remove dampness, and relieve pain. Used for conditions like wind-cold-damp pain in the lower back and knees.
Ngưu tất (Chen): Bitter, sour, neutral, and acts on the kidney and liver meridians. It helps promote blood circulation, strengthen tendons and bones. Used for lower back pain, knee pain, and muscle fatigue.
Tang ký sinh (Chen): Bitter, neutral, and acts on the kidney and liver meridians. It strengthens the kidneys and liver, improves tendons and bones, and clears the meridians. Used for joint pain and peripheral nerve pain.
Uy linh tiên (Zuo): Bitter, slightly warm, acts on the bladder meridian. It helps dispel wind, remove dampness, and relieve pain. Used for joint pain, muscle cramps, and difficulty moving.
Đan sâm (Zuo): Bitter, slightly cold, acts on the heart and liver meridians. It promotes blood circulation, relieves pain, and calms the mind. Used for blood stagnation, chest pain, insomnia, and irritability.
Chỉ xác (Shi): Bitter, spicy, slightly cold, acts on the spleen and stomach meridians. It resolves Qi stagnation, helps with phlegm, and treats indigestion.
Phòng phong (Shi): Pungent, sweet, and slightly warm, acting on the lung, spleen, and bladder meridians. It expels wind, relieves pain, and treats joint pain and muscle spasms.
Trần bì (Shi): Bitter, spicy, slightly warm, acts on the lung and spleen meridians. It regulates Qi, strengthens the spleen, and helps clear phlegm. Used for digestive problems and excess phlegm.
Xuyên khung (Shi): Pungent, slightly warm, acts on the liver, gallbladder, and heart meridians. It promotes Qi circulation, relieves pain, and dispels wind. Used for headaches and pain due to wind-dampness.
Tế tân (Shi): Pungent, warm, acts on the heart, lung, liver, and kidney meridians. It dispels wind-cold, activates water metabolism, and clears blockages. Used for cold-induced pain, dampness, and blood stagnation.
This formula works to dispel wind, warm and remove cold-dampness, nourish the liver and kidneys, and clear the meridians to reduce pain. It treats both the external causes of the disease and strengthens the internal organs, providing a holistic approach to treating sciatica, improving bone health, and promoting overall body strength.
